At Onward Home Health Care, we deliver CFSS services through the agency model, ensuring care is tailored to each person’s needs and respectful of their cultural background. 

What is CFSS?

CFSS stands for Community First Services and Supports. It is a Minnesota program that helps people live safely and independently at home. CFSS began on October 1, 2025, and is replacing the older PCA (Personal Care Assistance) program. 


How is CFSS different from PCA?

  • CFSS gives more flexibility in how care is arranged.

  • Some family members, including spouses and parents of minors, may now be provide care.

  • A budget can also be used to buy certain goods and services that help with independence and safety, like Personal Emergency Response Systems (PERS).

  • People receiving services can help write their own plan, or get support to create one.

 

 Who can receive CFSS services?
Anyone person who is eligible for Medical Assistance and requires support with activities of daily living tasks such as bathing, dressing, transfers, mobility, IADLS, toileting, grooming, eating assistance, and positioning. The program is designed for people of all ages who want to remain at home and be part of their community.

Why is CFSS important?
It gives individuals and families greater independence, flexibility, and assistance in shaping care. The program allows services to be arranged in ways that reflect each person's lifestyle, culture, and personal goals, making it easier to remain at home and stay connect to community life.
