Personal Care Assistance (PCA)
As of October 1, 2024, the Minnesota Department of Human Services (DHS) is transitioning from Personal Care Assistance (PCA) to Community First Services and Supports (CFSS). This change ensures participants continue receiving the same essential supports—such as help with daily living activities, health-related tasks, household support, and behavior redirection—while gaining more flexibility and choice in how services are delivered.

Through CFSS, individuals may choose the Agency Model, where care is provided by a licensed agency like Onward Home Health Care, or the Budget Model, which gives participants more independence in hiring and managing their own staff with support from a Financial Management Services provider.

At Onward, we are here to make this transition smooth. We are still accepting participants who are currently in the PCA program and will support their transition to CFSS at their next assessment. Our goal is to ensure your care continues without interruption while giving you greater control and more options for the future.

PCA to CFSS Transition FAQ's
What happens if I’m still receiving PCA services?
If you are currently on PCA, you do not need to worry. You will continue receiving services until your next annual assessment. At that time, your services will move to CFSS.
Do I need to apply again to get CFSS?
No. If you already receive PCA, you do not need to reapply. Your eligibility and services will be reviewed during your next scheduled assessment, and you will be transitioned to CFSS at that time.
Will my services change under CFSS?
Depending on your assessment evaluation completed by your county, your services may stay the same or be adjusted to fit your current needs. The main difference with CFSS is that it's designed to give you more flexibility and choice in how your care is provided.

Can I stay with my current provider?
Yes. Onward Home Health Care is accepting new CFSS participants, including those currently receiving PCA. We will work with you, your family, and your case manager to make sure your transition is smooth.
